On August 8, 1997, Health Horizons, Inc. ("Health Horizons") filed a complaint for fraud against State Farm Mutual Automobile Insurance Company ("State Farm") in the DeKalb Superior Court. In paragraph 3, Health Horizons mistakenly alleged that it "is a Georgia corporation."
