In May 1992, a fire damaged Zandra Perkins's home in Murray, Utah (the Murray fire). After notifying her insurer, Bear River Mutual Insurance Co. (Bear River), and after meeting with a claims adjuster, Perkins hired First General Services, Inc. (First General) to perform the repair and reconstruction work.
