Gary Harrison, an employee for Union Tank Car Company, mistakenly hooked an air-driven spot grinder to an unlabeled oxygen line and began working inside a tank car. The oxygen ignited, killing Harrison and causing fellow employee Richard Kevin Carter to be severely burned and injured.
