Vijay Jacob was injured in the course of his employment with Fox Custom Cabinets (Fox). At the time of Jacob's injury, Columbia Insurance Group (Columbia) was the workers' compensation insurance carrier for Fox. Jacob brought this action against Columbia and the Nebraska Second Injury Fund to recover benefits he claims to be entitled to for injuries he sustained to his left hand, left arm, and neck.
