L.D. Reynolds instituted a claim for personal injury against Dr. J. Bruce Steigner, Hospital Service District No. 1 of the Parish of Terrebonne (Terrebonne General Hospital), Dynatech Corporation and Louisiana Medical Mutual Insurance Company. Pursuant to La.R.S. 40:1299.47 Reynolds filed a request for review of the claim with the Commissioner of Insurance (Commissioner).
