Plaintiff, Donald Cormack, instituted these proceedings against defendant, the Prudential Insurance Company of America, to recover the sum of $1,653.00 allegedly owed by the defendant to the plaintiff under a group accident and health insurance policy issued by defendant to the employees of Union Carbide and Carbon Corporation and their dependents.
