Plaintiff alleged that, in consideration of the payment of a premium of $28.25, defendant issued to him a policy of insurance providing for disability benefits including loss of time by sickness; that the policy was issued on June 1, 1950; that plaintiff became disabled because of sickness in April 1951, and unable to perform and carry on his usual occupation; and that defendant has refused to make payments of monthly installments of $100 as provided...
